Job Description

Develops, monitors and documents individualized mental health treatment goals and objectives for each patient as part of the interdisciplinary treatment plans.

Provides evidence-based individual, milieu, group, and family psychotherapy, as well as other clinical therapeutic intervention in accordance with the patient's interdisciplinary treatment plan and the behavioral health program.

Schedules and coordinates one to two weekly individual sessions with the patient and one family session with the guardians, unless otherwise established by the interdisciplinary treatment plan.

Works under the guidance of the Program Director to establish a caseload and weekly group coverage schedule

Provides timely and adequate documentation of all sessions and patient encounters.

Provides staff members formally and informally with de-escalation techniques, psychoeducation and other effective clincal strategies for caring for individuals with behavioral and medical issues.

Participates in regularly scheduled team conferences with the interdisciplinary team for all patients on caseload.

Provides ongoing assessments to identify and address behavioral concerns that are interfering with medical or physical rehabilitation progress.

Communicates and coordinates intervention with the interdisciplinary team to achieve cohesion and effectiveness in a specialized psychiatric, medical and physical rehabilitation inpatient program.

Effectively communicates and provides accuate updates to the Program Director and Program Medical Director regarding treatment planning, behavioral programming, clinical coordination, goals, objections, areas of clinical concern, progress and the services successfully implemented.

Provides recommendations and actively participates in discharge planning and transition from the hospital to the next level of care.

Professionally presents self when representing the facility.

Staying current with the latest research and practices in the behavioral management and care of patients with behavioral health concerns as it applies to the role of the psychological treatment team.

Participates in ongoing program development meetings to assess the efficacy of current programs and makes recommendations for treatment fidelity.

Performs other duties as assigned.

EDUCATION:

Master’s degree required.

EXPERIENCE:

Training and experience in developing, training, implementing, and monitoring written behavioral programs.

Minimum two (2) years working in an inpatient hospital/rehab setting preferred.

LICENSURE/CERTIFICATION:

LPC, LMSW, or LCSW.
