Nurse LPN

Duties: In this role, you will practice community-based nursing, administer medical care and provide clients with basic medical information. Serves as a liaison with physicians and other client providers.

Our Location: We are located in Mount Vernon, Ohio, a short 30-minute scenic commute from Columbus, Zanesville, Lancaster, and Newark.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Employees are expected to perform essential duties to performance standards. Practices community-based nursing and administers medical care.

Provides clients with basic medical information, promotes healthy lifestyle change including tobacco cessation, nutrition, physical activity, substance use prevention and medication adherence.

Serves as a liaison with physicians and other client providers. Coordinates with client’s physician regarding client’s medical care and communicates with physician regarding client’s mental health condition. Coordinates with physicians, psychiatrist, and pharmacy regarding client’s prescriptions.

Administers shots and daily medications by following prescribed orders.

Completes nursing progress notes and annual nursing assessments.

Documents client’s vital signs including blood pressure, weight, BMI, waist circumference and other data.

Works effectively as part of interdisciplinary team providing assessment and referral to match clients with appropriate type and level of care.

Maintains confidentiality of HIPAA and other confidential and sensitive information.

Maintains the highest level of integrity and professionalism in all aspects of job performance.

Keeps abreast of current trends and “best practices” in the field. Attends and participates in required trainings on topics relevant to the job and to behavioral healthcare.

Performs job responsibilities and maintains all records in accordance with BHP’s policy, procedure and protocol, and as appropriate to the job, all regulatory authorities’ accreditation/licensure/ Certification standards, and all applicable laws and regulations.

Works assigned schedule, exhibits regular and punctual attendance and works outside of normal schedule as required.

OTHER DUTIES AND RESPONSIBILITIES

Performs other duties as assigned.

Qualifications: Candidates must possess strong written and oral communication skills and the ability to collaborate with other service providers. Associate’s degree in Applied Science of Nursing and three to five years of nursing experience in a facility for individuals with mental health issues. Licensed Practical Nurse (LPN) and CPR/BLS required. Basic computer, phone and typing skills are necessary for all positions.
