Position Summary:

The Care Management Ambassador will play a vital role in promoting and facilitating enrollment in our care management programs across various clinic locations. Tasked with engaging patients in clinic waiting rooms, the Care Management Ambassador educates individuals about the clinical and social benefits of participating in our care management services. Through effective communication and personalized interactions, the Care Management Ambassador assesses patient needs, provides information, and assists with enrollment processes, either on-site or through referral channels.

Primary Duties & Responsibilities:

Engage with patients in clinic waiting rooms to introduce and explain the benefits of our care management programs.

Educate patients about the clinical and social advantages of enrolling in care management, emphasizing personalized support and improved health outcomes.

Assess patient's needs and eligibility for care management services through effective communication and active listening.

Utilize informational materials and resources to effectively convey the value proposition of our care management programs to patients.

Assist patients in completing enrollment forms on-site, ensuring accurate and timely documentation of their interest in participating.

Facilitate referrals to Care Harmony via Veradigm patient messaging for individuals unable to enroll on-site, ensuring seamless transition and follow-up.

Maintain up-to-date knowledge of care management program offerings, eligibility criteria, and evolving healthcare trends to address patient inquiries and concerns effectively.

Demonstrate a commitment to patient-centered care and ethical conduct, prioritizing confidentiality, respect, and empathy in all interactions. Qualifications

Qualifications:

Previous experience in care management, care coordination, case management, or a related healthcare role.

Knowledge of healthcare delivery systems, including primary care, specialty care, and community-based services.

Strong interpersonal skills and the ability to build rapport with diverse populations, including patients, families, and healthcare professionals.

Excellent communication skills, both verbal and written, with the ability to convey complex information in a clear and concise manner.

Empathy, compassion, and a patient-centered approach to care delivery.

Ability to prioritize tasks, manage time effectively, and work independently in a fast-paced environment.

Urology or surgical experience preferred but not essential. Knowledge, Competencies & Skills:

Collaboration and teamwork skills to engage with multidisciplinary care teams and community partners.

Understanding of care management principles, including care coordination, continuity of care, and population health management.

Demonstrated ability to work independently and as an effective healthcare team member.

Proven organizational skills and ability to prioritize effectively.

Excellent customer service skills.

Strong written and verbal communication skills.

Proficient computer software and database skills.

Strong attention to detail.

Ability to multi-task, effectively problem-solve, and work in a fast-paced environment.

Must have the ability & willingness to travel to other sites as necessary within a geographic region. Physical Requirements for the Job:

Regularly required to sit and stand for extended periods.

Involves standing, sitting, walking, bending, stooping, and twisting.

Requires full range of body motion, including manual and finger dexterity and eye-hand coordination.

Occasionally lifts and carries items weighing up to 40 lbs.
