The Care Access Nurse supports the needs of patients/families being admitted to services by ensuring clinical information is adequate for a safe and effective admission and to facilitate obtaining clinical record details to support eligibility for home health or hospice services. Acts as a resource for referral sources and patients/families being admitted to Chapters services. Operates with a sense of urgency to ensure speed and accuracy to facilitate timely admission to services while adhering to Organizational expectations of productivity and efficiency. This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
