RN Case Manager

RN to provide home care visits in patient's homes in the Kokomo and Logansport area. Mostly in Kokomo at this time. Office hours will also be required at times. Assist with wound care, patient assessments, supervisory visits, developing Plans of Care, and other home care tasks.
