Position Summary:

Provide guidance and support to individuals impacted by Alzheimer’s disease and other dementia and respond to disease-related questions/situations by providing guidance, problem-solving, decision-making support, crisis assistance, and connections to local services. Conduct clinical assessments and action plans that identify needs and offer interventions based on those needs, including assistance with future planning, and in-depth education about the disease and caregiving.

Responsibilities

Essential functions and responsibilities include, but are not limited to:

Build rapport and engage constituents utilizing clinical techniques to determine appropriate interventions and resources.

Handle high emotions, trauma, and grief of constituents with calmness and empathy to assess their current situation and take the next appropriate step(s).

Help constituents better understand Alzheimer’s disease, the progression, and the needs of people living with the disease.

Provide accurate and efficient documentation for interactions including the presenting issue and goal, interventions, and action plans.

Teach multiple caregiver support classes.

Teach multiple early-stage memory loss classes.

Link callers to education, support, local community resources, and emergency/crisis services if needed.

Follow established Care Consultation policies, procedures, business rules, and clinical protocols.

Uphold the Memory Matters policy on client confidentiality.

Achieves grant deliverables as assigned.

Qualifications

Bachelor’s degree is required. Experience in the field of aging, and/or dementia and social services is preferred. Master’s degree in social work or related field, State Social Work licensure, and/or Certified Dementia Practitioner certification is preferred

Knowledge, Skills, and Abilities

Proficient clinical/counseling skills in listening, assessment, empathy, collaboration, goal setting, and action planning.

Ability to handle interactions with constituents involving heightened emotions including crisis, abuse, suicide/homicide risk.

Attention to detail, effective time management, and multi-tasking skills

Familiarity with healthcare systems, mental health, aging and disability services, and Alzheimer’s disease/dementia resources.

Experience with solution-focused brief therapy.

Proficiency in Microsoft Office and Google Apps, including video conferencing.

Fulltime: Exempt 37 + hours a week.
