Interim Healthcare of Palmdale is a Hospice Agency in the Antelope Valley looking for a Full-Time Community Liaison for Hospice - LVN, to join a family-oriented company, owned and operated by Saint Andrew’s Abbey. General Purpose:

Grow profitable revenues from Interim HealthCare Hospice of Palmdale by effectively developing and executing sales and marketing strategies to referral sources and prospective clients.

Essential Functions:

· Establishes and maintains working relationships with referral sources such as doctor's offices and other health care facilities.

· Enhances the perception of Interim HealthCare of Palmdale Hospice.

· Prepares sales plan with Executive Director/Administrator, including forecasts, target accounts, contacts within accounts and budget for achieving objectives.

· Create and implement account development strategies to target, nurture and grow accounts.

· Causes profitable growth by establishing and maintaining contact and relationships with referral sources through which prospective clients might be reached and influenced.

· Increases awareness of and interest in Interim HealthCare Hospice of Palmdale by preparing and conducting presentations regarding Interim HealthCare Hospice to organizations where prospective clients might be reached or influenced.

· Participates in strategic planning through analysis of assigned territory.

· Enhances customer satisfaction and loyalty by meeting or calling selected clients and family members and periodically conducting patient satisfaction and referral source satisfaction surveys to identify and address any issues.

· Develops and implements marketing plan for Hospice Services.

· Assures effective conversion of inquiries by working in conjunction with the Executive Director/Administrator to establish and monitor the process to track and respond to Interim HealthCare Hospice of Palmdale inquiries.

· Monitors and recommends actions to improve process through which inquiries are converted to sales.

· Meets with potential Hospice clients and family to explain hospice benefits and solutions in such a manner that they opt for the hospice benefit through Interim HealthCare Hospice.

· Meets with appropriate managers of the consumer audience, skilled home health services, and other appropriate facilities, to assure optimal conversion of appropriate Interim HealthCare clients and patients to Interim HealthCare Hospice.

· Completes other assignments as requested and assigned.

· May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.

Minimum Education & Experience Requirements:

· Active and in good standing LVN license in the State of California.

· Working knowledge of and experience in marketing/sales.

· Three (3) plus years experience in selling hospice or health care services.

· Demonstrated effective written and verbal communication skills, including the ability to communicate with all levels of management within and outside the company.

· Demonstrated familiarity with hospice or health care services, referral sources and payors.

Knowledge, Skills & Abilities Required:

· Computer and Internet literacy.

· Demonstrated excellent organizational and detail skills.

· Demonstrated ability to accomplish goals under short deadlines, changing priorities and minimal resources.

· Proven ability to work independently with minimal supervision.

Working Conditions & Physical Effort:

· Ability to work flexible schedule and/or evening hours as needed.

· Able to engage in local and occasional out-of- town travel.

· Physical activity is light requiring occasional lifting/carrying up to 20 lbs. or pushing/pulling small objects. Frequently walking or standing or sitting most of the time.
