JOB OPPORTUNITY - Admissions and Case Manager, Secure Treatment Program - Full-Time Permanent

Position Summary

The Admissions and Case Manager will ensure that Youthdale values are reflected in the ongoing triage of services to identified need, transition, and continued recovery of the children, youth, and young adults. This role will over see the admission pathway to our Secure Treatment program.

The Admissions and Case Manager will work closely with other members of the multidisciplinary team in assessing clients' mental health status, formulating treatment plans, implementing planned care and treatment, and evaluating outcomes. Works in partnership with clients, caregivers, and staff of relevant community and regulatory agencies; participates in planning and developing mental health services and provides community mental health education.

The position reports directly to: Director, Intensive Services

Supervises the Admissions and Case Coordinator, Secure Treatment

Corporate Responsibilities

Fosters positive interpersonal relationships with the staff, clients, visitors, outside agencies, government officials and related professional associations.

Maintains and adheres to all relevant legislation, organizational policies and procedures.

Works in compliance of the Occupational Health & Safety Act and its regulations, reporting hazards, deficiencies and contravention’s of the Act, in a timely manner.

Demonstrates professional practices, which are consistent with the mission, vision and philosophy of Youthdale Treatment Centres.

Job Responsibilities

Lead the operation of all intake functions as it relates to court-ordered and emergency admissions to the ASU and other units as required

Gather collateral and referral information to present at prioritization or bed flow meetings, and facilitate a discussion to ensure acceptance decisions are made for clients who meet the CYFSA criteria for secure treatment

Must utilize standardized tools to aid with admission/decline decision making such as the CALOCUS and INTER-RAI

Collaborate with client and caregiver to ensure their voice is incorporated in the process

Assist with determining the type of admission to the ASU: court order or emergency admissions

For court ordered admissions, assist caregivers with the paper work required to obtain a court order, and assist with securing funding if needed

For clients who are declined - ensure they are triaged to appropriate services

Liaise with legal counsel as required for court ordered and emergency admissions

Is the lead on attending court

Ensure documentation standards are met and data collection as per policy guidelines

Provide case management services to coordinate and support appropriate services for clients and their families while at Youthdale

Identifying, developing and coordinating appropriate discharge plans as needed for clients by collaborating with internal and external stakeholders

Verbal communication and writing documentation that addresses client’s care and treatment. Coordinating meetings and appointments as mandates and/or needed

Consult with inter-professional teams to determine client needs

Working collaboratively with client and caregivers to identify and implement appropriate interventions

Represent Youthdale in a professional manner during external community meetings

Ensure standards of professional practice with respective regulatory bodies (Ontario College of Social Workers and Social Service Workers, MCCSS, MOH, Auditor, Accreditation, etc.) are met

Maintain and adhere to all relevant legislation, organizational and occupational health and safety policies and procedures, and the mission, vision, and philosophy of the agency.

Understanding the CYFSA and ministry licensing requirements as it relates to Child and Youth Services

Facilitates clients' utilization of appropriate community services and rehabilitative programs by performing duties such as researching available services, liaising with these agencies, providing related advice and consultation to clients and providing referrals to appropriate community treatment alternatives and services. Utilizes available community resources to develop supportive and rehabilitative programs for adults and their families.

Maintain clients records and prepare reports

Provides support and direction to clients requiring enhanced case management by monitoring their mental health status and assessing their social adjustment and ability to perform routine life activities, including managing money and taking medication regularly. Provides follow-up care and services as required, including providing referrals to other healthcare professionals such as Recreational Therapists, Occupational Therapists, and Psychiatrists.

Review Board – provide oversight to review board processes

Qualifications

Master’s degree in Social Work or Psychotherapy.

Regulated Health Care Professional and registration with the OCSWSSW, or related professional association

At least four years of relevant experience such as intake/triage/case management

Knowledge of community resources within the GTA and step down interventions

Knowledge of children and youth mental health and the social determinants of health.

Expert knowledge of system navigation and referral pathways to community mental health, addiction support, health, social agency resources, etc.

Strong interpersonal and communication skills.

Ability to communicate verbally and write clear, concise reports and documentation.

Clinical experience is required to support decision-making, i.e., escalation or changes in service levels.

Experience with multi-disciplinary collaboration/communication.

Experience facilitating transitions between treatment services

Experienced with standardized triage tools and assessment tools

Driver’s license. Travel is required.
