RN CONFIDENTIAL Director Admissions - LTC Nursing Home - Manhattan NY

A recognized healthcare organization in New York City is actively seeking an experienced, licensed Registered Nurse (RN) to join their staff as a Director of Admissions.

Responsibilities:

The Director of Admissions will:

Coordinate admissions for nursing and rehab center for all new patients and returning patients to ensure all required clinical information is available to the Admitting Nurse

Complete daily census, including upcoming discharges and admissions

Create and handle Primary Care Physician tracking reports for incoming patients to open lines of communication between facility and physicians offices

Maintain client relations to increase admissions to the facility

Tour family members within the facility and coordinate with heartland health care hospital liaison for new admissions to the facility

Coordinate team meetings, take minutes, and update members, as necessary

Create documents and reports to assist in keeping team members informed

Assist with events planning for the facility and outside events where the facility is involved

Maintain processes within the facility to ensure the continuation of communication between departments as they pertain to admits and discharges

Ensure admissions agreement and contracts are signed by new patients in a timely manner and consequently filed appropriately in the business office

Perform other duties, as needed Qualifications:

Bachelor's Degree in Nursing

NYS Registered Nurse (RN) license

Experience with Admissions in a Nursing Home

Experience working in a Nursing Home at Director level Desired Skills:

Master's Degree in Nursing
