Job Description

We are seeking a compassionate and skilled Registered Nurse to join our team. As a home health RN, you will play a crucial role in delivering essential medical care to patients in their homes. Your dedication to providing high-quality, compassionate care and meticulous documentation in the Electronic Medical Records (EMR) system will significantly impact our patients' well-being and recovery.

Responsibilities:

Patient Care and Assessment:

Perform thorough patient assessments to create and execute individualized care plans.

Administer medications, wound care, and specialized treatments as prescribed.

Monitoring and Education:

Monitor patient conditions, offer education, and support patients and families in managing illnesses and recovery.

Provide guidance on healthy practices, medication management, and disease prevention.

Collaboration and Documentation:

Coordinate care with the interdisciplinary team, ensuring holistic patient well-being, and meticulously document patient progress and care provided in the EMR system.

Maintain accurate and timely documentation to ensure comprehensive patient records.

Emotional Support:

Provide emotional support and empathy during challenging periods of illness and recovery.

Advocate for patients' needs and preferences, ensuring their well-being and comfort.

Adaptability and Problem-Solving:

Adjust care plans as needed, applying critical thinking and problem-solving abilities in varied home settings.

Address unexpected situations or emergencies effectively and efficiently.

Role Requirements:

THESE ARE THE PRIMARY REQUIREMENTS YOU MUST PASS THESE REQUIREMENTS TO BE CONSIDERED: - WOUND VAC EXPERIENCE, PICC LINE EXPERIENCE, OSTOMY CARE EXPERIENCE, CATHETER CARE EXPERIENCE, AND KINNSER?WELLSKY EXPERIENCE.

Education: Registered Nurse (RN) license in good standing and completion of a nursing program from an accredited institution.

Experience: Prior experience in home health or a related field is preferred.

Compassion: Demonstrated empathy, sensitivity, and a patient-centered approach to care.

Communication Skills: Excellent verbal and written communication for effective patient and family interaction.

Adaptability: Ability to work independently and adapt to different home environments while ensuring patient safety.

Ethical Conduct: High ethical standards, maintaining patient confidentiality, and respecting their autonomy.

Technical Skills: Proficiency in utilizing Electronic Medical Records (EMR) and other healthcare software.

Driver's License: Some roles may require a valid driver's license and reliable transportation for home visits.
