Urologic Specialists of Northwest Indiana has an exciting opportunity for a Nurse Cancer Care Coordinator to join our growing practice and provide high quality, professional care and support to our patients.

The Nurse Cancer Care Coordinator will serve as the main contact for our cancer patients and coordinates the various aspects of care. The Nurse Cancer Care Coordinator will also be responsible for infusions as directed by the physicians.

Key Responsibilities:

· Serves as the main contact for cancer patients and their family members about their disease and treatment plan.

· Provides appropriate education and guidance to patients and family members on symptom management and complications of therapy and when to contact the care coordinator.

· Works with patients to ensure all required lab and diagnostic testing is completed timely. Communicates lab results to physician as required.

· Actively identifies and addresses barriers to care that might keep the patient from receiving timely and appropriate treatment for their diagnosis.

· Administers specialty infusion therapies as ordered by the physician.

· Conducts initial and ongoing patient assessment and ensures patients comfort during infusion treatment.

· Works directly with pharmacy for therapy drugs.

· Collects patient/treatment data and enters statistical information into tracking system.

· Assists in urological procedures as delegated by the physician.

· Interacts respectfully with the patients and their families. Treats patients with compassion and dignity.

· Identifies patients’ educational needs and intervenes appropriately to meet those needs.

· Utilizes sound judgment in the establishment of priorities in patient care management along with the clinical team.

· Documents patient data, actions or interventions, responses, and plan for care/follow-up according to guidelines and standard practice.

· Identifies and recommends clinical trials for practice participation.

· Coordinates and implements clinical trials within the practice.

· Creates and maintains SOP’s for the clinical trials.

· Follows patient care throughout the clinical trial.

· Performs other duties as assigned.

Required Skills/Abilities:

· Excellent patient assessment, critical thinking, and problem-solving skills required. Excellent attention to detail required.

· Strong verbal and written communication skills required.

· Maintains infection control practices.

· Ability to demonstrate compassion and caring in interacting with others.

· Strong decision making and practice skills.

· Ability to act with integrity, professionalism and confidentiality.

· Proficiency with Electronic Medical Record. Epic experience preferred.

· Working knowledge of Excel.

· Ability to work in a fast-paced environment.

· Updates knowledge and skills through continuing education.Requirements:

Education/Experience:

· Registered Nurse with a minimum of two years of nursing experience, preferably in urology and/or oncology preferred.

· Infusion experience preferred.

· Previous research experience and/or clinical trial experience preferred.

· State of Indiana Licensed Registered Nurse in good standing.

· Basic Life Support Certification (BLS)
