*Highland Hospital is seeking a Part Time APP to join its Geriatrics Home Care team. Candidates for this role will work three (3), eight (8) hour shifts per week, Monday - Friday, Days. The exact days are yet to be determined.
Some duties include but are not limited to:
As a Geriatric Advanced Practice Provider (APP) within our Home Visit Program under the supervision and guidance of the Medical Director(s) of Geriatrics, you will be responsible for delivering comprehensive medical care to geriatric patients in the comfort of their homes as well as performing telehealth visits. Working closely with the interdisciplinary team, you will assess, diagnose, and manage the healthcare needs of our elderly population, ensuring high-quality and patient-centered care.
Proven experience in providing direct patient care in geriatric healthcare settings, preferably with home visit experience. This position requires you to work independently, prioritize tasks, and adapt to changing environments. Excellent communication, interpersonal, and team collaboration abilities. Strong clinical assessment, diagnostic reasoning, and care coordination skills. Commitment to compassionate, culturally sensitive, and person-centered care for older adults. Salary Range:
$93,000.00 - $119,642.00 annually
*Based on a 40 hour work week, prorated to match FTE
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Job Requirements
Education: Fulfilled the requirements to be a licensed PA or NP in NYS.
Experience: Experience preferred but new graduates will be considered
License/Certification Required: NYS License for PA candidates. Current, valid license to practice as an NP in NYS for NP candidates. Must be BLS certified. NYS Drivers License required.
Skills: Knowledge of various diagnostic and therapeutic procedures recommended
