Physical Therapist

Summary:

This position will entail treating geriatric and disabled young adult patients in their home environment based upon their needs for physical therapy. All necessary equipment will be provided as well as adequate instructions on each individual patient and their needs. Evaluations and treatments will be regular practices for the Physical Therapist to conduct.

Responsibilities:

Point based system of 7 points daily to be considered full time

Intakes, recertifications, follow-ups and skilled visits

Responsible for opening and closing all patient visits

Documentation Systems/ EMR: OASIS for Medicaid and Medicare and AccessEMR

Requirements:

Graduate of an accredited school of Physical Therapy.

Possession of a Connecticut State License

BLS Certified.

1yr Home Care experience preferred Benefits/Perks:

13 week assignment, extensions offered if need exists

Local contract under 50 miles = $45-$50/hr

Travel contract over 50 miles = $2022/wk gross (net of $1950/wk from tax free stipends)
