Physical Therapist - Home Health

Join our client's Home Health team as a Physical Therapist and make a meaningful impact on patients' lives. In this role, you'll deliver exceptional Physical Therapy services directly to patients in the comfort of their homes. Key Responsibilities:

Conduct Physical Therapy assessments and develop personalized treatment programs

Document and report patients' responses to treatment and changes in condition

Educate patients and caregivers on therapy programs and device usage

Supervise physical therapy assistants and, when needed, certified nursing assistants

Prepare comprehensive reports and actively participate in staff and agency development activities

Schedule and Shift Details:

Monday to Friday; 8:00 am/9:00 am to 5:00 pm

One weekend per month (Saturday & Sunday; day shift with scheduled patients)

Two holidays (one summer, one winter); no on-call duties

Travel:

Local travel to residential homes

This is a pay per visit position

Requirements

Master's degree or higher in Physical Therapy

Minimum 1 year of experience in a healthcare setting

Valid Physical Therapist license in the state of Illinois (IL)

Valid regular driver's license
