Clinical Director (RN) for Home Health Agency

Our Clinical Director at Active Home Care is in charge of leading a developing group of clinical experts in maintaining the health and wellbeing of the customers in the neighborhood.

If so, you are the person for the job. Active is currently seeking a Clinical Director (CD) to join our team! As CD, you will have primary responsibility for leadership to ensure that our clients and staff are well supported. In this role, you will develop written plans of service to be delivered to our clients and then supervise the delivery of the services by your clinical team through review of client records and client supervisory visits.

Primary Responsibilities (including, but not limited to):

As clients join AHCS, the CD will review referrals for approval, assess the needs of each client, and specify services that shall be delivered based on a written plan developed in collaboration with the client, the client's family, and orders (Plan of Care). The CD will work with leadership to build and develop policies and procedures to support the clinical staff at Active Home Care.

Administer nursing care to ill, injured, or disabled patients.

Diagnose and establish patient treatment plans.

Monitor and report changes in patient symptoms or behavior.

Communicate with collaborating federal and state bodies as well as physicians or specialists regarding patient care.

Educate patients and families about health maintenance and disease prevention.

Facilitate referrals to other healthcare professionals and medical facilities.

Maintain accurate patient medical records.

Provide advice and emotional support to patients and their family members.

Supervisor and coordinate clinical staff.

Develop and maintain clinical policy and procedures.

Requirements

RN (Required. Must be active and unrestricted in the state of MD or compact)

Previous experience in home health (beneficial) nursing or other medical fields

Familiarity with medical software and equipment

Ability to build rapport with patients

Strong problem solving and critical thinking skills

Ability to thrive in a fast-paced environment
