Overview Monterey Park Hospital located in the San Gabriel Valley of Los Angeles, is a 101 bed acute care community hospital.

Monterey Park Hospital has provided our community with advanced medical and culturally sensitive healthcare services for over 50 years.

Monterey Park Hospital is currently recruiting for a healthcare Discharge Planner to work in our Utilization Review Department.

This position reports to the Director of Managed Care Responsibilities The ideal Discharge Planner candidate has experience managing the discharge/transition process by working closely with the patient and/or family and coordinates activities with the multidisciplinary team including physicians, nursing staff and community resources to ensure the patient has an adequate postacute care transition.

The Discharge Planner supports our physicians by providing necessary information to referral agencies and keeping physicians informed of readiness for discharge from a standpoint of required post-discharge services.

This position assists patients and their families in coping with psychsocial strengths and weaknesses in the context of illness and treatment plans and adjustment to an altered lifestyle.

The Discharge Planner maintains documentation of all services in appropriate charts, summaries and logs.

Performs related duties as assigned.

Qualifications Education: High School Education/GED or equivalent preferred.

Associate's/Technical Degree orequivalent combination of education/related experience preferred.

Experience: 2 years' experience working in an acute care facility or similar healthcare positionpreferred.

License/Certifications: Basic Life Support (BLS) required.

Other: Demonstrated knowledge of Medicare/Medi-Cal and HMO Health Plans required.

Demonstratedgood communication skills, both verbal and written English, required.

Bilingual English/Spanish toeffectively communicate with patients and families preferred.

Ability to facilitate and leadinterdisciplinary rounds.
