Position Overview: Under the supervision of the Cohort Manager, the Reentry Site Supervisor (RSS) is a public health worker who oversees the Reentry Little Baker team and provides highly complex case coordination to participants fully engaged in a holistic reentry process.

The RSS will lead and supervise the coordinated care team, train new staff members, provide logistical guidance and support, and maintain a working knowledge of and compliance with ACCESS Harris objectives and procedures.

The RSS will primarily supervise at an all females secure facility-Harris County Women's Empowerment Center.

The RSS will also oversee outreach efforts to increase access, communication, community engagement, and education for justice-impacted residents in and out of the Harris County Women's Empowerment Center.

This position will provide case management and transitional management to persons currently incarcerated and persons who have been released from incarceration and need a continuum of care.

* This is a grant-funded position* Duties and Responsibilities: * Oversee operations/activities of the Reentry Little Baker team; provide direction to employees by organizing, maintaining staffing levels, ensuring new employee training, performance management, and participating in regular meetings; assess program progress and continuously improve team performance and program development.

* Oversee operations/activities of participants' care-coordination team; responsible for providing ongoing trauma- responsive case management services for participants enrolled in ACCESS Harris; work with participants to enhance stability and overall well-being by addressing specific needs that may have contributed to the identified vulnerability while acting according to standards and laws in emergencies involving the health and safety of participants being compromised.

* Assist with internal and external referrals for medical, substance use, mental health, parenting, employment, housing needs, and other needs related to social determinants of health.

* Primary point of contact for ACCESS Harris is the Reentry Cohort Manager and care coordination team; provides continuous feedback to the Care Coordination Team during case conferences; maintains case records and documentation as appropriate.

* Responsible for work-flow facilitation through the effective use of the care coordination team, increasing productivity and efficiency to meet ACCESS Harris program initiatives.

Also, for maintaining a professional working relationship with the Harris County Sheriff's Department-Women's Empowerment Team.

* Conduct site/home visits as needed to complete initial assessments on new participants and provide follow-up to existing participants; ensure the safety and basic needs of the participant are met in an appropriate and caring manner.

* Monitors status reports, metrics, or ad hoc reports, as requested by leadership; develops and maintains knowledge of operations and functions of ACCESS Harris and related services.

* Support participant engagement in services, including following up with external service providers including but not limited to physical and mental health, substance use treatment, parenting support, and educational programming; help participants make informed decisions by acting as their advocate regarding their clinical, emotional, nutrition, and life skills status; intervenes with treatment options when necessary.

Requirements Education: * Bachelor's degree in criminal justice, social or behavioral sciences or an administrative field.

Experience: * Two (2) years' experience working with individuals impacted by incarceration.

* One (1) year experience working as Case Manager or in Case Management.

* One (1) year working with justice impacted individuals.

* Two (2) years of supervisory experience leading teams to reach challenging goals and objectives in the social service space.

Licensure/Certification: * Valid Driver's License (TX upon hire). Work Environment: * This job primarily operates at an all-female secure facility (Harris County Women's Empowerment Center). * This role routinely uses standard office equipment such as computers, phones, iPad, photocopiers, filing cabinets, and fax machines.

* Staff may make site visits using personal vehicle.

* Staff in this position should possess a high level of tolerance for this emotionally demanding and stressful work while at the same time being supportive and understanding of the stresses being experienced by both participant and staff.

Physical Demands: * Frequently required to sit or stand, use manual dexterity, speak, listen, hear, and write.

* May occasionally lift and move up to 25 pounds and occasionally lift to 50 pounds.
