Clinical Dietitian
Part Time
3 Days/Week
Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years. The Clinical Dietitian will play an integral role in helping us to achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care
About the role:
As the Clinical Dietitian, you will have direct responsibility for the nutritional care of residents on the assigned units as well as monitoring the effectiveness and outcomes of medical nutrition therapy and interventions. You'll be a critical member of the healthcare team as it relates to all areas of medical nutrition therapy.
Schedule:
9:00 AM - 5:00 PM
Part Time 3 Days/Week (no weekends)

The Ideal Clinical Dietitian:
Bachelor's degree in Food and Nutrition, Dietetics, or Food Management from an accredited university
Active membership in the American Dietetic Association
NYS Registered Dietitian Certification strongly preferred
Prior experience in clinical dietetics and supervision in a healthcare facility, hospital, or long-term care facility.
Primary responsibilities include, but are not limited to:
Interviews and screens admissions, readmissions, return transfers, and families to obtain diet history and develop diet plans.
Develops and implements nutritional care plans that identify the needs of residents while considering religious and cultural food preferences.
Evaluates resident's prescribed diets, review food intake
Instructs and counsels residents and families regarding prescribed diets and documents
Attends and participates in Comprehensive Care Plan and Discharge Planning.
Maintains ongoing communication with physicians, nurses, and other Interdisciplinary Team members regarding the residents' nutritional care.
Supervises resident's meal service on units for tray accuracy, feeding problems, food quality, etc.
Prepares and presents in-service and other training sessions.
