SUMMARY OF POSITION AccessHealth Anderson is a program to assist qualifying low-income adults without health insurance in Anderson County.

AccessHealth brings together a network of healthcare providers and community agencies to ensure that those in underserved communities have access to quality of care in the right place at the right time.

The Community Health Worker (CHW) provides healthcare navigation services to enrolled patients, coordinates medical home placements and first appointments, and collaborates with a multi-disciplinary team to address the social determinants of health.

The CHW provides health coaching and advocacy to improve healthcare utilization and assists enrollees with applications for healthcare payor options, prescription assistance and/or other benefit programs as appropriate.

The CHW will be expected to attend appointments and make home visits as required to advocate for the enrollee and to connect them to needed resources.

The CHW will work with many different types of partners including health care providers, managed care plans, human service organizations and community-based organizations.

This position remains knowledgeable of community resources and is apt to share, educate, and dissemination appropriate information to assigned communities.

The CHW would develop trusting relationships through the communities and community partners to provide culturally appropriate health education, provide resources, social support, informal counseling, and advocate for individuals and community needs.

EDUCATION/QUALIFICATIONS Required Qualifications include:# High school diploma or equivalent, required.

Current SC CHW certification required (or obtained within one year of employment) Preferred Qualifications include: Any combination of three (3) years health/social services or applicable community experience Bilingual speakers, specifically Spanish, highly encouraged to apply Bachelors or Associate Degree, preferred.# SUMMARY OF POSITION AccessHealth Anderson is a program to assist qualifying low-income adults without health insurance in Anderson County.
