Who We Need

We’re looking for someone with the following skills, experience and credentials:

Completes the initial ISP and service delivery plan based on specified health, wellness and behavioral health goals. Collaborates with the new service recipient, family and/or significant others to implement an effective service plan, explaining the available clinical options.

Provides screenings and evaluations for new referrals, including counseling, residential and supportive services and community resources. Makes recommendations for initial authorization of service referrals. Completes initial referrals in accordance with goals from the ISP.

Completes intake packet, including, but not limited to: consents, member handbooks, notice of privacy practices, releases of information, etc.

Provides education of and assists service recipient in Patient Portal enrollment.

Orients service recipient to the outpatient site, team, services, etc. and provides education to T19 recipients regarding on-site Primary Care services.

Serves as the initial point of contact for new service recipients via telephone or face-to-face interactions with a person, family or other involved party for the purpose of facilitating new enrollment.

May be required to assist clinical team with routine, yearly comprehensive assessment and Integrated Service Plan (ISP) completion in order to ensure contact compliance expectations are met.

Coordinates and communicates with Clinical Director, Clinical Coordinators, care team, nursing and medical and psychiatric practitioners for individuals exhibiting urgent or emergent medical and/or psychiatric symptoms.

Pursues best practice outcomes for persons with mental illness including continuing education, employment, independent housing and community tenure.

Consistently meet organizational encounter and performance goals and follow clinical documentation standards as defined by company policy.

Job requirements

Bachelor’s degree in behavioral health related field, and (2) two years of full-time work experience in behavioral health.

Equivalency of a HS Diploma/GED and (6) six years of full-time behavioral health work experience acceptable.

Bilingual (English/Spanish) preferred

Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of

Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
