University Hospital Social Work and Case Management Administration

Case Management is a team process that includes assessment, planning, facilitation and evaluation of the services required to meet the patient’s needs through the healthcare system. The Case Management Extender will provide programmatic support services for Case Management & Social Work covering patient’s in UH, Ross, Brain and Spine, Critical Care Tower, East Hospital and Dodd Hall. The Case Management Extender will be responsible for coordinating CM/SW program support inclusive of throughput/capacity management, discharge planning, regulatory document provision, and CM/SW extender functions at the direction of the Case Manager and/or Social Worker for selected teams.

Minimum Qualifications

Bachelor's degree or equivalent combination of education and experience in assisting with patient care operations. Experience in Acute Care Hospital Setting preferred. Proficient in Microsoft Office (Word, Excel, Power Point, Outlook). Experience with EPIC preferred.
