Job Description
North Suffolk is looking for a Care Navigator to join our Emergency Logan Airport/Co-respond Team! The Co-respond team services Logan Airport by providing education, case navigation and transition assistance for individuals arriving through Logan Airport. The Care Navigator works with Mobile Clinicians, Massport, and the Massachusetts State Police with engagement and outreach directly on site at Logan.
This position is on-site at Logan Airport with some evenings and/or weekends required depending on program schedule.
What you'll do:
Support the day to day operations of the Co-respond Team located at Logan Airport
Assist with coordinating referrals by communicating fully and effectively with referral sources to facilitate prompt and comprehensive responsiveness.
Coordinate with collateral providers and community partners such as health care providers, outpatient behavioral health providers, recovery coaches, etc.
Perform referral, outreach and engagement activities with program participants, including follow-up appointments for mobile crisis visits.
Under the supervision of a manager, identify risk and behavioral crisis needs and bring to the attention of clinical staff and or State Police as appropriate and as needed.
Meet individually with program participants as indicated to complete intake paperwork, and address administrative or collateral communication needs, etc.
Maintain and update databases and spreadsheets, as directed, to assist with tracking clinical information, collateral communication, client assignments, etc. .
What you'll need:
Education:
Bachelor's Degree in Social Work, Counseling Psychology, Expressive Therapies, or related field required
Experience:
Two years of post-degree experience
Skills:
Excellent communication skills
Ability to complete documentation in EHR systems (such as Epic &eHana)
Ability to work with a multidisciplinary team consisting of Clinicians, Massport, and the Massachusetts State Police
