Geriatric Nurse Practitioner

Job Description

Seeking either a full time or part time Nurse Practitioner to work with an established geriatric practice in Westchester County, NY. Geriatric certification is preferred however will consider an adult/family medicine trained clinician with geriatric experience. You will work with a board certified geriatrician to provide primary care services for the residents of assisted living facilities.

A full time hire will work 3-4 days per week in the field and 1-2 days working from home. You also will receive 4 weeks of paid time off.

A part time hire will work 1-2 days per week in the field.

Whether you are a full time or part time hire, you will be sent to the facility with a nurse to assist you in seeing your patients. The practice utilizes a very intuitive and user friendly EMR that is web based. You will be given a computer and a mobile WIFI to assist you in performing your visits.

Primary Responsibilities:

Conduct assisted living facility visits of residents to function as their primary care provider.

Communicate with family members or HHAs in order to keep care givers up to date on patient needs and care plan.

Follow up on diagnostic tests ordered such as blood work and radiology studies

Order services such as visiting nurse and physical therapy.

Required Qualifications:

Must have current New York State license as a Nurse Practitioner

Must have experience in geriatric primary care

Nurse Practitioner Certification from either the ANCC or AANP with Geriatric Certification or experience in geriatrics.

Must be vaccinated against COVID-19
