Overview: Trident Regional Medical Center in South Carolina is seeking a skilled and experienced Director of Case Management to join their team. This full-time position offers a rewarding opportunity to oversee departmental activities, ensuring compliance with organizational policies and external regulatory agencies. The successful candidate will play a pivotal role in coordinating patient care and discharge planning processes, optimizing clinical resource utilization, and achieving established outcomes through interdisciplinary collaboration.
Responsibilities:
Direct and evaluate departmental operations, including staffing, utilization of information technologies, and staff competencies.
Ensure adherence to care coordination and discharge planning processes, with a focus on timeliness, quality, and documentation.
Collaborate with key stakeholders, including the Facility CFO, Division CM Leader, CMO, CNO, and ancillary service leaders.
Assess and improve departmental performance through data analysis and implementation of performance improvement activities.
Establish and maintain working relationships with key stakeholders to facilitate effective case management initiatives.
Qualifications:
Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred.
️ Either Registered Nurse (RN) licensure or Licensed Clinical Social Worker (LCSW) required.
️ Minimum of 5 years of clinical experience, with at least 3 years in overall acute care Hospital Case Management and 2 years in case management leadership.
Strong leadership, communication, and presentation skills.
Demonstrated ability to achieve goals and deliver results in a healthcare setting.
