The Hospice House Clinical Supervisor supports the clinical operations of the Hospice House as well as delivers age appropriate care to Hospice House patients when needed. The Clinical Supervisor manages by planning and prioritizing, organizing work, keeping daily operations functioning, setting attainable goals, and making decisions based on the mission and services of the agency. The Clinical Supervisor will supervise Registered Nurses, Licensed Practical Nurses, and Licensed Nursing Assistants (LNAs) and other Hospice House employees as assigned. The Clinical Supervisor is the clinical expert and resource for all Hospice House clinical functions and processes.

Essential Functions

Provides clinical supervision oversight to nursing and LNA staff at the Hospice House according to federal and state regulations.

Evaluates and redesigns processes and work flows to optimize quality, efficiency and utilization of resources.

Identifies improvement opportunities, has a thorough knowledge of all clinical Hospice House functions and acts as a support and resource to clinical staff.

Actively participates in the QAPI program, performance improvement projects and medical review.

Coordinates Hospice House patient referrals and patient care with the Medical Director and attending providers ensuring that patient needs are met and continuously assessed according to the individual needs of the patient and physician ordered Plan of Care.

Assures compliance with all state, federal laws regarding licensure and certification of the agency. Maintains knowledge of the Medicare Conditions of Participation for Hospice and an understanding of NH Hospice Administrative Rules.

Facilitates IDG meetings with staff to ensure that multi-disciplinary cases plan of care, goals and discharge plans are aligned and coordinated between team members.

Monitors clinical documentation to ensure compliance with agency policy and procedures, regulatory and accreditation requirements.

Interviews and recommends new candidates for hire.

Coordinates supervisory visits of nursing staff in accordance with agency policies and procedures, regulatory and accreditation requirements.

Completes performance evaluations for direct reports at the end of the introductory period and annually.

Participates in identifying and planning for nursing and LNA clinical competencies.

Ensures that competencies and required education is completed timely.

Oversees and assists with scheduling to ensure adequate nursing and LNA staffing for the Hospice House.

Grants time off in accordance with agency policy and while ensuring adequate clinical staffing to meet patient care needs.

Receives and documents reports of incidents, complaints and grievances according to Policy and Procedure.

Orients, coaches, mentors and precepts new nursing and nursing assistant employees.

Covers nursing shifts as needed for staffing gaps and shortages as needed.

Demonstrates flexibility in the role by accepting other duties as assigned for which qualified.

Requirements

Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).

Three to four years of recent nursing experience with two years in a hospice or palliative care setting.

One to two years supervisory experience or relevant work experience.
