Implementation of physician\xe2\x80\x99s prescribed plan of care

Patient assessments and coordination of care\xc2\xa0\xc2\xa0\xc2\xa0

Administration of prescribed medication, treatments, and therapies\xc2\xa0\xc2\xa0\xc2\xa0

Compliant, timely documentation of care provided in system of record\xc2\xa0\xc2\xa0\xc2\xa0

Health promotion, teaching, and training of family members\xc2\xa0\xc2\xa0\xc2\xa0

Responsible for accepting and writing verbal orders from physicians as required and for obtaining co-signature of Case Manager on all verbal orders\xc2\xa0

Follow up with physician, client, family members, staff, and Case Manager on client problems, concerns, and/or changes in client condition\xc2\xa0\xc2\xa0\xc2\xa0\xc2\xa0

Performs other appropriate duties as assigned\xc2\xa0

Qualifications\xc2\xa0\xc2\xa0\xc2\xa0\xc2\xa0

Must have and maintain an active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice\xc2\xa0\xc2\xa0\xc2\xa0

Current CPR certification from American Red Cross, American Heart Association, or BLS\xc2\xa0\xc2\xa0\xc2\xa0

TB skin test (current within last 12 months)\xc2\xa0\xc2\xa0\xc2\xa0

Six months prior hands-on nursing experience preferred but not required\xc2\xa0\xc2\xa0\xc2\xa0

Must have reliable transportation\xc2\xa0\xc2\xa0\xc2\xa0
