Qualified Professional-Community Support Team

Job Description

Seeking a Full time (part time may be considered) Qualified Professional (QP) for our Mecklinburg County area Community Support Team. The QP will be responsible for assisting the Team Lead in keeping all documentation updated for submission of authorizations. Indirect and face-to-face visits with a caseload of up to 36 clients on the team will be needed. Attendance to the monthly Clinical and Family Team meetings will also be part of the job duties. The QP will supervise the Certified Peer Support Specialist on the team and all will be supervised by the Team Lead. QP will attend weekly team meetings to ensure that client's needs are being met as well as the team is running efficiently and smoothly. QP will assist in linking clients with community resources and assisting the Peer Support Specialist to do so as well. QP will keep PCP treatment plans updated and will review them monthly with clients and establish new goals if needed. QP will assist in helping to keep the CST team running efficiently and smoothly for the benefit of the clients.Company Description
