We are a private substance abuse treatment center looking for a Case Manager to eliminate any barriers to patients remaining in treatment. We are small, intimate, and a family type treatment center looking for a permanent addition to our wonderful team. The ideal candidate is organized, humble, tactful, punctual, and coachable. The ideal candidate also has strong documentation skills and is a self-starter.

The Responsibilities include:

Demonstrates proficiency in utilizing community resources as supportive services.

Serving as a liaison for the client, family, referral source and other social service agencies.

Presents in a professional manner in the community, in person, on telephone and in written communication including times of extreme stress and when others are upset.

Works effectively with culturally diverse groups and individuals experiencing an exacerbation of either psychiatric, substance use and/or medical symptoms, as well as experiencing a stressful situation.

Works cooperatively in the work environment and takes direction from management with dignity, respect and in a productive, professional manner.

Works cordially with others in a team setting.

Assumes a professional, helpful style in all encounters with visitors, vendors and other professionals and represents the company in a positive manner 100% of the time.

Consistently demonstrates an understanding of, supports, and focuses on the vision, mission, goals and objectives of the company in day-to-day activities.

Ability to render compassionate care and an equal commitment to serve others in need of assistance.

Reviews and responds to electronic and handwritten mail, memoranda and other correspondence and responds to phone inquiries timely (within 24-hours or next available business day) and in a professional manner.

Ability to report to work as scheduled, in a timely fashion and will an attitude have oriented toward service.

Must have knowledge and must comply with state and federal statutes, rules and policies that affect the target population.

File Short-term disability and FMLA

Complete assessments and notes for each session with patients

Complete weekly individual case management sessions

Schedule outside appointments and setup transportation

Ensure all medical follow up is completed and any necessary documentation is uploaded into the patient chart

Complete Prior Authorizations for services or medications as soon as initially indicated, and subsequently prior to expiration, in order to ensure continuity of care.

Must have excellent organizational, time management, and writing skills

Complete other tasks as assignedLANGUAGE SKILLS: Ability to read, write and speak in English and Spanish (or other languages) helpful. Must be able to read and comprehend simple instructions, short correspondence, and memorandums. Agility to effectively communicate information both orally and written on one-to-one and small group situations to clients and other employees.

ORGANIZATIONAL SKILLS: Must be organized and able to schedule workload appropriately without hindrance to client appointments etc.

DECISION MAKING: Must be able to work independently with competent decision making using established criteria and guidelines.

Job Type: Full-time This is a Monday-Friday position 9:00am-5:00pm.

A minimum of one years experience in a case management position, working with a consumer population appropriate to the available position is preferred. Experience can be waived for individuals with appropriate field or volunteer experience.
