Community Support Team Lead

Job Description

Alamance Academy is a mental health and substance abuse agency seeking a full-time team lead for our Community Support Team. A part time team lead will be considered with the right conditions that can be discussed further. Community support team works with clients who have mental health and or substance abuse issues and are typically in a housing program and getting ready to be transitioned for independent living. The team lead will be responsible for conducting therapy for the mental health clients as well as seeing each client on the team at least once per month face-to-face. The team lead will also supervise the other staff on the team and conduct weekly team meetings. Team lead will be responsible for completing clinical and family team meetings once per month for all clients and responsible for documentation such as therapy notes, team meeting notes and supervision notes. They will also be responsible for completing any assessments and addendum‘s in an effort to get clients re-authorized for more services. Services will be conducted both in the community, in the office and work from home/remote. Team lead will need to be either fully licensed or a licensed associate with either an LCSW or LCMHC.
