Overview:

AHRC Nassau provides OPWDD-approved Day Habilitation Services for adults with IDD in over 30 locations across Nassau County, Long Island. We offer both traditional, site-based day habilitation services in a community-based setting, as well as Program Without Walls (PWW) options. PWW provides an additional opportunity for men and women to learn new skills, build relationships and develop self-esteem through volunteerism outside of the traditional setting. All day habilitation services are designed to support each person in discovering their personal interests, while improving their communication and socialization skills. The services are designed to include a special focus on opportunities for building successful and meaningful interactions with other members of our community.

Responsibilities:

Primary duties and responsibilities include but not limited to:

Develops individualized habilitation plans in conjunction with the program volunteers and ensures all required documentation is appropriately completed and maintained

Coordinates appropriate on-site and community-based activities for volunteers

Directly supervises Assistant Manager and DSP's

Oversees and, when needed, participates in individual transports to and from the program, as well as to and from volunteer locations and other community based activities

Maintains communication with parent/correspondent and appropriate service coordinators of volunteers regarding progress, problems, volunteer needs, and all pertinent information that needs to be disseminated

Provides direct support services to program volunteers as needed

Provides oversight and assistance to volunteers and visitors in the care of animals (horses, goats, etc.) including feeding, grooming, maintaining of specialized equipment, stables, etc.

Provides oversight and assistance to volunteers and visitors in maintaining and cultivating the grounds and horticultural initiatives

Ensures protection of volunteers from neglect/abuse during program hours; reports incidents to administration in a timely fashion

Appropriately documents all physical plant issues, as well as ensures all furniture, equipment, and supplies are properly ordered and maintained

Attends meetings, conducts observations, and consults staff as a means to discuss, review, and gauge each volunteer’s progress, problems, and individual needs

Trains and supports volunteers to engage in tasks related to functional/meaningful and/or volunteer job assignments/responsibilities to the best of their abilities at primary agency, community volunteer jobs, farm animal care, ground maintenance, and horticultural programming

Promotes and reinforces all agency philosophies including the guiding principles

Successfully completes and maintains all required trainings/certifications determined by the needs of the program volunteers/site

Performs additional job related duties as requested by supervisor

Ensures that work-related safety procedures are in place and are being adhered to by staff and reports any observed or discovered unsafe act, or potentially hazardous situation, to the Program Coordinator and/or safety committee

PHYSICAL DEMANDS/WORK ENVIRONMENT

May include, but are not limited to, lifting and transferring of an adult participant. Transportation/driving as needed, inclusive of wheelchair vehicle/lockdowns.

Qualifications:

High school diploma or equivalent required. Bachelor’s degree in Human Services related field preferred.

Minimum one (1) year experience supporting people with intellectual and other developmental disabilities preferred.

Good oral and written communication skills and computer proficiency in all Microsoft Office Suite applications preferred.

Qualified NYS driver license and must be maintained during employment.

AMAP/CPR/First Aid/SCIP-R and defensive driving training preferred.
