Job Description
Position Title: Clinical Social Worker - Palliative Care Services
Qualifications:
Masters Degree in Social Work from an accredited institution
Must hold a license in the state of practice as a Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW)
New graduates and out-of-state MSW-prepared Social Workers are expected to obtain their state license within 90 days of hire
Full-time work experience in palliative care and end-of-life care is required
Demonstrated experience working with diverse and high-risk populations
Responsibilities:
Provide clinical services to patients and families, addressing psychosocial, environmental, age-specific, and cultural issues to optimize emotional, social, and physical well-being and efficient use of healthcare and community resources
Conduct bio-psychosocial assessments, intervene, and provide treatment as necessary
Operate with a high level of organization, independence, and initiative, identifying areas for improvement and assuming responsibility accordingly
Adhere to HIPAA and Joint Commission standards, following the BMC Mission, Vision, and Values
Consult, assess, and intervene with high-risk patients and families regarding emotional and social issues in palliative and end-of-life care, utilizing clinical social work techniques and theory
Collaborate with BMC staff and colleagues to ensure comprehensive patient care, considering structural factors affecting health
Develop thorough bio-psychosocial assessments tailored to patients with chronic and terminal illnesses and their families, considering age-appropriate and cultural factors
Assess the needs of patients, families, and caregivers across physical, psychological, socioeconomic, spiritual, and symptom management domains
Utilize the palliative care departments psychosocial assessment tool as a guideline for intervention planning
Identify and address barriers and risk factors, including systemic issues that may hinder symptom management and patient/family care
Implement a variety of clinical interventions, including psychotherapy, counseling, crisis intervention, care coordination, and advocacy for access to services and resources
Participate in initial palliative care consults, providing psychosocial consultation on patient care planning and management
Recognize and intervene in issues of loss, adaptation, and bereavement along the illness continuum
Facilitate patient-family-team meetings and report suspected cases of abuse or neglect, ensuring comprehensive consultations and legal compliance
Maintain thorough documentation of patient interactions and adhere to hospital confidentiality policies
Work independently and collaboratively within interdisciplinary teams, serving as a liaison between palliative care and social work services
Attend scheduled meetings and educational programs to maintain competency and uphold hospital standards of conduct
Additional Duties:
Provide guidance and instruction on psychosocial dynamics to interdisciplinary colleagues
Maintain licensure through continuing education units
Seek assistance from colleagues with specialized knowledge as needed
