Responsibilities

Complete initial and ongoing assessments of all individuals of 18-person caseload

Assess patients’ conditions as needed

Send chart observations, and perform evaluation tasks including vital signs and medication reviews

Assist PA with monthly clinics on site

Oversee medication administration by unlicensed personnel

Assist with quarterly Psychiatric clinic

Ensure appropriate medical services are provided for all IDD individuals

Assist with medical appointments

Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Complete Workers Compensation paperwork and follow up as needed

Qualifications

North Caolina state license as a LPN

Proof of current CPR certification

Compassionate, friendly demeanor

Excellent communication skills

Current driver’s license and reliable transportation

Ability to work a flexible schedule

On call availability
