Job Description: Hospice Case Manager

Position Overview:

The Hospice Case Manager is responsible for coordinating and overseeing the care of hospice patients and their families. They serve as a primary point of contact, providing comprehensive support and guidance throughout the hospice care process.

Primary Responsibilities:

Conduct initial and ongoing assessments to determine patients' hospice needs and develop individualized care plans.

Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and spiritual counselors, to deliver holistic care.

Provide skilled nursing care, including medication management, symptom control, and wound care, as needed.

Coordinate services and resources to meet patients' physical, emotional, and spiritual needs, ensuring continuity of care.

Offer emotional support and counseling to patients and their families, addressing their concerns and facilitating difficult conversations.

Educate patients and families on hospice philosophy, treatment options, and end-of-life care planning.

Document patient assessments, care plans, and interventions accurately and timely in accordance with regulatory standards.

Communicate effectively with healthcare providers, caregivers, and community resources to optimize patient outcomes.

Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.

Adhere to ethical and legal standards, maintaining patient confidentiality and respecting patients' autonomy and dignity.

Qualifications:

- Bachelor's degree in nursing (BSN) from an accredited institution (preferred).

- Current registered nurse (RN) licensure in the state of practice.

- Minimum of 1-2 years of clinical experience in hospice, palliative care, or a related healthcare setting.

- Strong assessment, communication, and critical thinking skills.

- Ability to work independently and collaboratively in a fast-paced, interdisciplinary environment.

- Compassionate demeanor and a commitment to providing high-quality, patient-centered care.

- Proficiency in electronic medical record (EMR) documentation and computer skills.

- Valid driver's license and reliable transportation for home visits (if applicable).
