Position Description: Locum Tenens Nurse Practitioner (NP) or Physician Assistant (PA)
Location: Eureka, CA
Assignment Type: Long-Term Locum Tenens Contract with Extension Option
Hourly Rate: $95-120 per hour
Schedule: 4 days per week (Tuesday to Friday), totaling 0.8 Full-Time Equivalent (FTE)
Clinic Setting: Mobile Health Services/Telehealth and Visiting Specialist Center
Responsibilities:
Provide urgent and acute care services in an outpatient setting, including telehealth consultations.
Conduct mental health and substance abuse assessments and treatments.
Manage acute and chronic conditions, with a focus on chronic pain management.
Collaborate with visiting specialists to coordinate patient care.
See a mix of pediatric and adult patients.
Work independently and as part of a multidisciplinary team.
Requirements:
Current California NP or PA license.
Board Certification (BC) in Family Medicine, Urgent Care, or Acute Care.
Active DEA registration prior to start.
Experience in mental health and substance abuse care.
Proficient in chart closure within 72 hours of patient encounter.
Strong communication skills for telehealth consultations.
Additional Information:
No on-call responsibilities.
Credentialing process typically takes 30-90 days.
Decision on candidates made within 7-14 days of application.
Long-term contract with the option to extend based on performance and mutual agreement.
Work Schedule:
Tuesday to Friday, 8:00 AM to 5:00 PM.
1 day a week spent on the mobile van, exclusively seeing adult patients.
Expected patient volume of 28 per day in urgent care clinic and a minimum of 17 per day on the mobile van.

The compensation ranges from $95 to $120 per hour, with potential for extension based on performance and mutual agreement.
