Job Location: Onsite in Phoenix, AZ
Shift Options:
3pm-11:30pm - Wednesday - Saturday
5pm - 1:30am - Friday - Tuesday
11pm - 7:30am - Thursday - Monday
Pharmacist Overview:
This position is responsible for dispensing prescriptions for specialty drugs serviced at our local specialty site.
This position is responsible for interacting with health care professionals and delivering high quality, thorough medication counseling.
Must also transcribe prescriptions and perform clinical follow up.
Must be comfortable communicating with patients and prescribers by telephone and in person.
Other daily tasks will include prescriber clarifications, therapeutic interventions, and prescription entry.
Maintains patient/resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies)
Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated and discussed with appropriate members of the healthcare team
Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs
Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and policy
Informs the Pharmacy Manager of medication related errors
Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs
Pharmacist Qualifications:
AZ state license in good standing required.
Minimum of one year pharmacy experience in retail, clinical, or hospital setting preferred.
Experience with accredited Specialty Pharmacy preferred.
Basic computer skills required.
Ideal candidate will have strong interpersonal skills, be detail oriented, and have a positive attitude.
