Job Title: Physical Therapist Department: James SSCBC Oncology Rehabilitation Scope of Position Physical Therapy Rehabilitation and Lymphedema Team Member is a licensed physical therapist that works within the oncology team structure to offer outpatient rehabilitation and lymphedema services.

These services are offered at outpatient service sites to patients who have lymphedema and other oncology related rehabilitation needs.

Services will be provided at SSCBC as well as other James Ambulatory sites and may require travel during the work day.

The rehabilitation team members report to the Rehab Services Manager as part of the overall James Ambulatory Services structure.

Position Summary Each rehabilitation team member is first a part of a group effort to provide care within a continuous performance improvement environment, and secondly a distinct professional who provides specific contributions to the team approach.

Team members assess patients and establish together plans of care; provide a wide variety of treatment techniques; provide education and support to patients and families; and facilitate continuing care planning.

Team members evaluate quality of services and participate in outcomes management activities.

The Physical Therapy Team Member in rehabilitation and lymphedema in the SSCBC Oncology Rehabilitation Department will also have advance knowledge in the treatment of oncology patients and will be competent in the evaluation and treatment of lymphedema with complete decongestive therapy (CDT,MLD). Although team members report directly to the Rehab Services Manager, they are also responsible to each other, to those they supervise (physical therapy assistants and volunteers) and to other participants in patient care activities, including attending and resident physicians, nurses, their support staff, Social Work and Patient Care Resource Managers to assure coordination of services and programs.

Duties and Responsibilities 75% Clinical Practice Each rehabilitation team member provides services according go: the laws of the State of Ohio governing licensed professionals; standards of practice, scope of practice, clinical guidelines, and codes of ethics published by respective national professional associations; and OSUMC policies and procedures governing the work of licensed clinical professionals.

All staff must meet standards established by the SSCBC Oncology Rehabilitation department as part of the James performance appraisal system (P3). Core competencies include customer service, teamwork, job knowledge/skills, and resource management.

These core areas are integral to successful clinical practice.

Clinical Practice * Evaluates and provides treatment plan for patients per physician prescription.

Possesses knowledge base to select appropriate assessment; demonstrates ability to conduct comprehensive evaluations in the area of oncology and lymphedema.

Possesses skill in a variety of treatment techniques/procedures appropriate to the disorder and the patient's development and chronological age.

* Treatment plan reflects rehabilitation goals.

Documentation supports treatment.

Goals revised in reasonable time frames for goal attainment.

* Provides effective verbal and written communication to ensure team approach to rehabilitation.

Participates collaboratively and communicates with other team members of the interdisciplinary team.

* Provides effective and appropriate utilization of medical center and community resources and other team members (PCRM, Social Services, and Nurse Practitioners). * Actions promote positive relationships between staff, patients and guests.

15% Team Activities Team Activities are those that support care of the persons served, including scheduling, care planning sessions with patient and/or other team members.

Patient care conferences, family conferences, internal continuing education sessions, departmental/divisional staff meetings.

Team members also participate in clinical quality and outcome management activities.

In addition, team members may have opportunities to participate in medical center committee activities.

10% Professional Discipline Activities It is particularly important in an interdisciplinary environment that rehabilitation team members maintain formal links with members of their own profession, having opportunities to pursue discipline-specific continuing education, teach students and other professionals about aspects of practice, participate in research activities, and interact with members of their profession within and outside the medical center.

All team members will establish specific goals and activities, which relate to their professional discipline, including in-service education and periodic discipline-specific professional business meetings/conferences both within and outside the medical center.

Organizational Expectations Practices within the Medical Center's policies and procedures.

Adheres to the Intensive Caring Value statements as demonstrated through positive patient/guest relations, positive and effective interactions with staff, and formulating and meeting developmental goals.

Minimum Qualifications Currently licensed in the state of Ohio as a Physical Therapist.

Maintains professional licensure; demonstrates commitment to continuing education; meets mandatory education and health requirements; demonstrates competence in technical, interpersonal, and cognitive skills required to meet essential job functions.
