Substance Abuse Counselor
This individual will oversee many aspects of a client’s substance abuse treatment and lead that patient to a better life.
Essential Duties
· Conduct intake assessments on new patients, which include obtaining previous records that are relevant to the current treatment episode
· Assess the needs of the patient for counseling and level of care
· Providing individual and group counseling using Evidence Based Practices
· Providing crisis intervention as needed
· Ensure patient confidentiality
· Providing supportive counseling in conjunction with Medical and Nursing Services
· Contribute to the ongoing assessment and treatment planning process through participation in daily reports, case reviews, and treatment team meetings
· Maintain accurate and up-to-date case files on each patient per Federal, State, and organization requirements, which includes documenting all counseling and education services, assessments, re-assessments, referrals and follow-ups within the client’s clinical record
· Coordinate care between treatment providers, including physicians and mental health agencies, and any other relevant treatment provider as needed
· Provide coordination of support and referral services for the patient/family and document referrals within the clinical record
· Prepare unique and individualized treatment plans for each patient
· Develop patient discharge/aftercare/continuation of care plans
Minimum Qualifications
Licensure as a: CADC, LCADC, LAC, LPC, LSW or LCSW
