Dare to Care too is an expanding Multi Specialty Practice that serves the Maryland, Washington DC and Virginia area by providing a wide range of services including but not limited to the assessment and treatment of children ages six (6) and older and adults with physical and behavioral health disorders utilizing a comprehensive approach.

Department: Pharmacy

Reports to: Pharmacy Clinical Site Leader

Work Model: Onsite

Grade: D0:09

Hours: 12-18

Salary: (To commensurate with experience and consistent with UHN compensation policy)

Shifts: Variable

Status: Permanent part time

Closing Date: Start Date July 1st, 2024

Position Summary

The Staff Pharmacist provides comprehensive pharmacy services to patients within the practice where behavioral health, medical and weight management services are being provided

Duties

Review and dispense all medication orders for appropriateness

Identify, resolve and prevent drug therapy problems with the patient and health care team

Monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines

Conduct medication reconciliation

Follow-up on medication incidents and promote safe medication practices on the unit

Respond to drug information questions

Patient education

Provide direct and/or functional supervision of technician staff

Facilitate and support clinical teaching and research activities within the department, while exemplifying Dare to Care too's valuesQualifications

Possess a Bachelor's of Science Degree in Pharmacy, Doctor of Pharmacy degree or equivalent

Certificate of competency from the Pharmacy Examination Board of Maryland

Hospital Pharmacy residency, an asset; hospital pharmacy practice experience with patient-centered care

Excellent verbal and written communication skills

Excellent interpersonal skills

Excellent organizational and time management skills

We are seeking an optimistic and open minded individual who will thrive in a fast paced collaborative work environment to on July 1st, 2024. Work hours are between 10am-6:00pm Monday and Tuesday and 11:00am-8:30pm Wednesday and Thursday and Friday 10am-4pm

Starts at two days per week

Tuesday 10am to 1:00pm

Thursday 2pm-7pm

and increase as census does.

Compounding experience a plus
