We are seeking a Substance Abuse Counselor-ADT to join our team! You will be responsible for implementing effective treatment plans to clients in an individual or group setting.
Responsibilities:
Counsel and advise individuals or groups with substance abuse problems
Develop and implement individual treatment plans
Establish treatment goals and milestones with clients
Prepare progress reports and letters for relevant parties
Educate families and groups on substance abuse treatments and prevention programs
Facilitate referrals to other healthcare or counseling professionals
Qualifications:
Previous experience in counseling, social work, or other related fields
Experience in conflict resolution
Compassionate and caring demeanor
Ability to build rapport with clients
Excellent written and verbal communication skills
JOB SUMMARY
Substance Abuse Counselors must demonstrate the ability to develop an open, collaborative relationship with patients. The counselor must be proficient in the performance of the 12 Core Functions of an Alcohol and Drug Abuse Counselor and is required to be an active participant in the program's Continuous Quality improvement efforts. Responsible for providing alcohol and drug rehabilitation by engaging the client in individual counseling.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES
· Carries a client caseload as assigned. Responsible for determining the client's strengths/weaknesses and needs to assist clients in developing a treatment plan within 7 days of admission
· Monitors and documents assigned client's progress toward the treatment plan at least every 90 days using the treatment plan review and presenting it to the Clinical Supervisor for his/her review and signature
· Provides alcohol and drug education in counseling sessions
· Responsible for time management skills
· Responsible for discharging/transferring clients according to regulation
· Must keep client records up to date including transition planning
