Job Description
The Registered Nurse plans, organizes and directs homecare services and is experiences in nursing, with empathy on community health education/ experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities:
Administer nursing care to ill, injured, or disabled patients
Diagnose and establish patient treatment plans
Monitor and report changes in patient symptoms or behavior
Communicate with collaborating physicians or specialists regarding patient care
Educate patients about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to patients and their family members
Regularly evaluates patient nursing needs
Initiates the plan of care and necessary revisions as patient status needs
Provides health care instructions to the patient as appropriate per assessment and plan of care
Identifies discharge planning needs as part of the care plan development and implements prior discharge of the patient
Qualifications:
Registered nurse with current licensure to practice professional nursing in the state
Graduate of National League for Nursing accredited school of nursing
Maintains current CPR certification
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
Minimum two years of experience, at least one which is in the area of public health or home care nursing is preferred
Self-directed and able to work with minimum supervision
Demonstrates excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist
Shows ability to organize and prioritize workload independently
Responsible for supervising Home Health Aides
Prolonged or considerable standing or walking
Able to lift, position, or transfer patients. Able to life supplies and equipment
Previous experience in nursing or other medical fields
Familiarity with medical software and equipment
Ability to build rapport with patients
Strong problem solving and critical thinking skills
Ability to thrive in a fast-paced environmentCompany Description
