The Home Health Aide is a member of the home care team who works under the supervision of a Registered Nurse or Therapist and performs various personal care services as necessary to meet the patient's needs.
Essential Duties and Responsibilities:
Responsibilities of the Home Health Aide include, but are not limited to: baths; applying lotions to skin, oral hygiene, shampoos, changing bed linen
Assists patients with dressing and undressing
Performs skin care to prevent breakdown
Assists the patient with toileting activities
Keeps patient's living area clean and orderly, as appropriate
Plans and prepares nutritious meals
Assists in feeding the patient, if necessary
Takes and records oral, rectal or axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)
Assists in ambulation and exercise according to the plan of care
Performs range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)
Assists patient in the self-administration of medication per policy
Handles patient's laundry, as appropriate
Meets safety needs of patients and uses equipment safely and properly (foot stools, side rails, etc.)
Reports on the patient's condition and significant changes to the assigned nurse
Adheres to Amergis' documentation and care procedures and standards of personal and professional conduct
Minimum Requirements:
Certification may be required per state/contract
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age
