Our Community Support Workers (CSWs) provide community-based services to youth clients facing mental health and substance abuse challenges. This position involves direct client interaction to promote recovery and well-being through supportive services and outreach activities.

The ideal candidate will be deeply committed to nurturing long-term recovery and capable of working both independently and as part of a multidisciplinary team.

Primary Job Responsibilities:

1. Provide direct support to youth in community settings, assisting with daily living skills, medication adherence, and access to community resources.

2. Facilitate social and recreational activities to promote engagement and socialization among clients.

3. Accompany clients to appointments and assist with transportation needs to ensure they receive comprehensive healthcare and social services.

4. Work collaboratively with case managers and other health professionals to implement treatment plans.

5. Conduct regular check-ins and follow-up visits to assess client progress and needs.

6. Document client interactions, progress, and outcomes, maintaining accurate and confidential records.

7. Submit all documentation within 48 hours of completing the service.

8. Educate clients and their families about community resources and how to effectively navigate them.

9. Attend Supervision and participate in team meetings to discuss client cases and updates.

10. Participate in community outreach programs to raise awareness about mental health and substance abuse issues.

11. Provide crisis support and intervention under the guidance of licensed professionals.

12. Provide logistical support, including scheduling appointments and arranging transportation.

13. Perform administrative duties related to community support and client care.

Qualifications:

• High school diploma required; Bachelor's degree in Social Work, Psychology, or related field preferred.

• Experience in community work, particularly in mental health and substance abuse settings.

• Strong interpersonal skills with the ability to build rapport with clients.

• Knowledge of local community resources and services.

• Ability to work flexible hours, including evenings and weekends, as needed.

• Compassionate, patient, and dedicated to client well-being.

• Valid driver's license and reliable transportation
