Position Summary:

Assumes responsibility and accountability for the nursing assessment and/or primary care for a selected population of patients. Provides annual wellness assessments for patients in designated wellness clinics and in a variety of other settings. Maintains currency of licensure and certification. Functions under the collaborative practice agreement for delegated medical aspects of care with collaborating physician(s).

Essential Duties and Responsibilities include the following:

Provides annual health assessments for patients in a variety of settings such as in our Wellness Clinics and at the patient's home which include, but not limited to:

Obtain medical history and perform physical examination.

Complete review of medicines and related instructions.

Review home safety and psychosocial needs

Fulfill documentation requirements, timely.

Review all available medical records for each patient seen.

Maintain patient safety and confidentiality.

Universal precautions utilized in all patient care.

Provides anticipatory guidance, screening and health education to patients to promote and maintain health.

Assesses members for preventative interventions such as safety, fall risk, depression screen, dementia, and urinary concerns.

Home Visit Goal: 6-8 members per day

Wellness Clinic Goal: 12 members per day

Ensure Authorizations are submitted timely, if necessary.

Evaluates the chronic conditions of patients.

Instructs individual and/or families or groups about health or defined health problems.

Provides education to patient and family members based on screenings, assessments and the patient's chronic conditions.

Plans appropriate care based on screening and chronic care guidelines.

Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.

Flexible with the setting of the evaluations based on company need.

Promotes guest relations; anticipates and responds to the needs of others; provides assistance in a courteous, quick manner; treats others with care and respect maintaining privacy, confidentiality and dignity. Maintains a professional appearance and a clean, safe environment.

Participates with the company's Quality and Coding departments to ensure the requirements established by Centers for Medicare and Medicaid Services (CMS) are fulfilled for each patient.

Prep for each appointment by reviewing medical chart.

Assist with training and orientation of new personnel.

Identify problems, provide solutions and evaluate outcome.

Assist with the scheduling and notification of patients.

Communicate daily with assigned supervisor.

Document and maintain records within the company EMR on same day as patients are seen.

All other duties as directed by management

The pay range for this position at commencement of employment is expected to be between $125,000 to $130,000annually ; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements

Education and / or Experience:

Master's degree in nursing with advanced skills in physical assessment.

Minimum of three years clinical nursing experience and/or two years post masters clinical nursing experience desirable.

ANA Certification desirable within one year of employment.

Valid, unrestricted Registered Nurse Practitioner License in the State of CA.

State Nurse Practitioner Certification in appropriate specialty

or

Graduation from an accredited Physician's Assistant Program.

National Certification for Physician Assistant

Current California PA license in good standing

Current BLS certification.

1-3 yrs NP or PA experience with confidence to practice independently

Managed Care experience a plus.

Will travel to the Organization Locations.

Effective verbal and written communication skills.

Effective Interpersonal skills necessary to work productively with patient, families, nursing staff, and other departments.

Maintain collaboration agreement with contracted supervising physician
