This is a Per Diem (as needed) position working at Northwest Medical Center.
Position Purpose Provides assessment and crisis intervention in acute care settings; plans, organizes, and implements social work services for patients and families.
Provides long term care planning for patients/families.
Responsible for complex discharge planning.
Intervenes as needed with hospital patients with mental health problems or substance abuse problems.
Provides assessment with case management for pediatric patients.
Position Qualifications/Minimum Requirements Education: Required: Masters of Social Work degree Experience: Required: Two (2) years acute clinical experience Skills: Able to handle crisis situations that require judgment critical to patient welfare; Appropriate prioritization and planning skills, while achieving safety and quality in a high volume environment; Engaging and effective communication and service skills; Adherence to policy and procedures; Process improvement oriented; Self-initiative to assist with whatever is needed.
