SUBSIDIARY: Carson Valley Children's Aid PROGRAM: Philadelphia School District (PSD) Case Management JOB OVERVIEW: The Case Manager will be assigned to a Philadelphia School District school where they will provide support to referred students.

They will meet with each student and family to assess their needs, document a support plan, and ensure that the student is referred to resources to meet their needs.

In addition, they will continue to meet with the student for one year to ensure that there has been genuine engagement with resources and make additional referrals as necessary.

The Case Manager will be responsible for documenting and tracking all their activities and reporting them to the school staff.

EDUCATION/EXPERIENCE REQUIREMENTS: * Bachelor's degree in social work, education, or a related field * 1+ years of experience with children and families KNOWLEDGE, SKILLS, AND ABILITIES: * Ability to establish positive relationships with school officials, youth and families, and members of child-serving systems * Ability to effectively collect information and respond to questions from children/families and child-serving systems * Ability to communicate and interact with all team members * Computer literacy, including the ability to utilize pertinent software programs * Ability to accurately calculate numbers using addition, subtraction, percentages, etc.

* Familiarity with child-serving systems (e.g.

education, juvenile justice, child welfare, mental health, intellectual disability, drug and alcohol, healthcare, and vocational rehabilitation) * Problem-solving abilities with attention to detail * Ability to interact with people in varied settings * Ability to work both independently and cooperatively as part of a team * Ability to define problems, collect data, establish facts, and draw valid conclusions ESSENTIAL JOB FUNCTIONS: * Major highlights of the position requirements are noted below.

Additional responsibilities may be assigned.

* Engage with and assess the needs of students and their families.

* Develop a plan of care for each student and implement the plan.

* Conduct regular meetings with students over the course of a year.

* Complete tracking tools on a weekly basis and share data with school staff and supervisor.

* Participate in all school meetings.

* Maintain a caseload of twenty students.

Communication: * Maintain contact with school and family team members.

* Notify all participants of children/families' meetings and appointments.

* Clearly and effectively communicate pertinent information to responsible parties.

* Advocate for the needs of the children/families to accomplish care plan objectives.

* Develop and implement service coordination plans in conjunction with the treatment team.

Documentation: * Assemble all necessary documentation for submission to the funding source.

* Maintain accurate tracking documents as required by the funding source.

* Document contacts in the electronic health record.

Planning: * Schedule appointments and meetings in a timely fashion to ensure continuity of service.

* Work with families to anticipate and address any barriers to family participation in the service process.

TRAINING REQUIREMENTS: * Attend mandated orientation.

* Comply with the agency's annual training requirements.

* Participate in school district trainings as assigned.

WORK ENVIRONMENT AND SCHEDULE: * School setting * Monday through Friday * Hours are designated by school assignment.

* Must be available for all assigned activities outside normal working hours, when necessary to ensure quality engagement in new resources PHYSICAL CONSIDERATIONS: * Able to handle multiple assignments * Manages work-related stress * Demonstrates good judgment * Relates well to people * Possesses strong organizational and communication skills * Able to walk up and down stairs and lift 20 pounds EOE
