Salary:
To aid and assist the agency with the overall quality of the records of persons served and programmatic documentation. Oversee and assist with the delivery of comprehensive services to children, adolescents, adults and their families who have been impacted by serious mental illness/co-occurring disorders, which places them in need of support.

Primary Responsibilities:
Responsible for managing charts assigned to ensure their quality and
Manage and oversee all program clinical documentation (Individualized Services Plans, Monthly Reports, Quarterly Reports, Service Authorizations (Initial/Continued) psychological/psychiatric evaluations, discharge reports, signing, and double locking of notes) in accordance with DBHDS, CARF and Medicaid
Ensuring all important appointments gets scheduled (Psychiatric evaluations, Physical appointments, Outpatient therapy, psychological evaluations, meetings with schoolteacher) within the first 30-days of a individual receiving
Ensuring that all important chart documentation is obtained, scanned and uploaded properly (copy of psychiatric evaluation, copy of psychological evaluation, copy of school records, copy of medications, copy of physical, hospitalization records, discharge paperwork and copies of treatment info from other mental health providers) within the first 30-days of receiving services and throughout the duration of
Conduct all new service and continued service orientations and introductions with new and continuing
Manage all dates of important documentation (Individualized Services Plans, Monthly Reports, Quarterly Reports, Service Authorizations (Initial/Continued) psychological/psychiatric evaluations, discharge reports) via managing, creating and maintaining a “important date tracker” as well as updating service requirement dates on EMR
Print, call and fax authorization to release of information forms, call and follow up to obtain important documentation
Manage Service Coordination logs, Health information, Medication Information and Collateral Contacts of all individuals assigned to ensure accuracy and proper
Assist in the agency’s hiring, corrections, coaching and
Meet with staff as needed to address any concerns or outstanding
Responsible for conducting agency training to orientate and support the development of staff at
Conduct performance evaluations on their current
Completing billing with accuracy and in accordance with agency
Responsible for reviewing documentation of staff
Complete weekly counselor note report with accuracy and in accordance with agency
Responsible for maintaining original timesheet logs for counselors assigned
Assist the agency in the hiring and termination
Actively coordinates case management, discharge, and aftercare planning with referral
Engage in company audits monthly to ensure chart compliance of all charts
Responsible for obtaining direct hours weekly with individuals on caseload via (family sessions, quality assurance, individual sessions, fill-in sessions ).

Minimum Experience:
Must have a Qualified Mental Health Professional certificate provide by the department of Health Professionals.

Training:
Staff must be trained in Cardio Pulmonary Resuscitation, First Aid, Medication Administration training and TOVA before they begin
Additional Requirements:
Employee must be certified as being tuberculosis-free yearly
Employee must clear criminal background and CPS registry
Program coordinators are required to use their personal vehicles or the company vehicles to perform some of the duties assigned to their jobs. Therefore, at the time of hire individuals must provide proof of active driver’s license and insurance.
Should the individual receive a moving traffic violation at any time after employment they must notify their direct supervisor about the incident the next business day after receiving any violation(s). Further, the individual might be taken off the schedule in the event they have any lapse in automobile insurance coverage.
