This role provides clinical care including primary care, continuity care, acute care, co- management of patients with healthcare team, and participates in coverage of providers at Brown & Toland Health (“BTHealth”). This position will work closely with the clinical team and is primarily responsible for managing direct patient care with the implementation of evidence-based medicine and adherence to national and internal practice guidelines and policies. This position will also play a key role at driving clinical initiatives for BTHealth. 50+% of time will be spent seeing patients. The reminder will be spent doing a combination of Epic In Basket support for other clinicians and mentoring new Advanced Practice Professionals (“APPs”).

About Your Work:

In-Basket support for other BTHealth clinicians, including coverage when clinicians are out on PTO. This includes reviewing test results, responding to medication refill requests and responding to patient messages.

Mentor all incoming and new APPs

Clinical oversight on all questions/inquiries from APPs regarding patients and care. Will be available to consult with and offer support to APPs as needed.

Provides high quality care to patients in collaboration with the clinical team and BTHealth leadership.

Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.

May serve as the primary provider of care for patients as appropriate and based on scope of practice.

Collaborates with providers, ancillary personnel, community resources and other interdisciplinary team members to proactively resolve patient needs.

Leverages the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows and clinical compliance.

The Skills, Experience & Education You Bring

CA Licensed Nurse Practitioner

5+ years of Clinical Experience with 2+ of those years with Primary Care

Ability to function independently as well as part of a team in a dynamic environment and quickly adapt to changing circumstances and procedures.

Excellent communication skills and ability to adjust communication styles when working with patients, their families and colleagues.

Highly organized, strong time management skills and flexibility to manage multiple timelines and activities.

Ability to act as a mentor to new and incoming APPs.

Final determination of a successful candidate’s starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. The anticipated pay range for this role is listed in our salary posting for transparency but may vary based on factors including the candidate’s qualifications, skills, experience, and geographic location. This base pay range is specific to California and can vary based on Bay Area, Metro LA, and Greater California regions which may not be applicable to other locations.

Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
