Salary: $92,000 - $98,000 annually

SUMMARY:

Lead Clinic Registered Nurse (RN) is responsible for providing care in assessing patient problems and

needs. This is a combination of traditional ambulatory nursing as well as case management. He/she will

implement plans of care, advise patients on health maintenance and prevention, maintain clinical

documentation, mentor other staff, and/or provide case management per protocol under the direction of

the Provider. Bayview clinic in San Francisco is to be the primary work location, and candidate must be at

the Marin City location 1-2 days a week.

Areas of Responsibility:

Patient Care & Education

• Performs patient care activities that are assigned within the scope of competencies

• Collaborates with the Care team and Providers to increase understanding, involvement and to

enhance the MCHWC patient experience

• Performs comprehensive triage and advice (phone and walk-in) for adult/geriatric, young adult,

pediatric, infant and pregnant/nursing mothers under the direction of licensed clinical staff

• Processes medication refills in accordance with protocol

• Provides immediate response for urgent triage (includes phone) and codes within the facility

• Responds to and manages emergency codes as per protocol

• Maintains crash cart and oxygen tanks per protocol, including logging crash cart inventory and ordering

supplies monthly

• Performs nurse visits under the direction of Provider (s) (includes, but not limited to): wound care;

B12; Coumadin; Testosterone injections; Insulin and glucometer teaching; Depo; BP checks; suture

removal; medication reconciliation; and patient education for chronic disease per protocol

• Assists with various procedures

• Performs medication reconciliation and adjustments per protocol and with support of Provider

• Informs and educates patients on the significance of abnormal lab results relative to chronic disease

and/or Public Health tracked illnesses in accordance with MCHWC approved educational materials as per

protocol

• Effectively utilizes MCHWC approved educational materials to provide patient education for

medications, wellness, disease management, procedures and processes under the direction of

licensed clinical staff

Case Management

• Performs indirect case management (includes, but not limited to): patient recalls; missed cancer

screening care follow up; coordinating with specialty offices; and new patient intakes

• Provides direct case management for uncontrolled chronic disease, in accordance with MCHWC

policies and procedures, as well as provider discretion

Documentation

• Ensures a clean, orderly and functional work environment

• Effectively participates in problem identification and resolution

• Ensures proper care in the use and maintenance of MCHWC equipment and supplies

• Promotes continuous improvement of workplace safety and environmental practices

• Performs miscellaneous job-related duties as assigned

Training

• Assists and leads staff trainings

• Provides guidance to MA’s and oversees duties are performed efficiently, including directing flow as

necessary

• Mentors, trains, and orients all non-licensed staff as needed

SKILLS AND EXPERIENCE REQUIRED:

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty

satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability

required.

Education/Experience:

• Two year degree in Nursing required; Bachelors and/or Master’s degree is preferred

• Equivalent to an RN or higher degree from an accredited school of nursing

• One to two years of increasingly responsible nursing experience including experience in community

nursing preferred

• Demonstrated leadership skills and a desire to continually enhance nursing practice and

• Prepare clear and concise documentation and reports

• Apply basic time management and patient care coordination skills to effectively provide nursing

services

• Establish and maintain effective working relationships with those contacted in the course of work,

including service representatives and the general public

• Plan and organize multiple work tasks, problem-solve and analyze information toward sound

decisions

• Identify problems/issues, develop and evaluate options in order to implement solutions

• Appropriately manage highly sensitive and confidential patient information

Licenses and Certifications:

• Possession of an appropriate, valid California Registered Nursing License

• Possession of, or ability to obtain, an appropriate valid California driver’s license

• Possession of, or ability to obtain, an appropriate valid CPR certificate

• Possession of, or ability to obtain, an appropriate valid BLS certificate

• Candidate must be able to successfully meet MCHWC’s credentialing and privileging requirements

Language Skills: Ability to effectively present information and respond to questions and request s from

patients, co-workers, and others as necessary. Bilingual in English and Spanish, both written and verbal

preferred.

Mathematical Skills: Ability to add, subtract, multiply, divide in all units of measure, using whole numbers,

common fractions and decimals.

Reasoning Ability: Ability to recognize problems, collect data and establish facts.

Computer Skills: High level of computer literacy and proficient in MS Office (Word, Excel and Outlook),

electronic medical records and web applications. Types 60+ WPM.

• Movement within health center complex environment repeatedly throughout the day

• Ability to write by hand, use phone, use computer keyboard and mouse extensively to perform

extended periods of time

• Ability to effectively communicate by hearing continuously and speech in a manner which can be

understood by a diverse population

• Ability to give and follow verbal and written instructions with attention to detail and accuracy

• Must have visual acuity near and far (20 inches to 20 feet), depth perception, field of vision, ability

to focus on an object, ability to identify and distinguish colors

• Ability to sit or stand for extended periods of time (minimum periods of one (1) hour at a time)

• Ability to frequently walk, bend, squat, stoop, kneel, twist, crouch, and climb with the use of a step

stool

• Ability to grasp, hold and pick up and reach with hands and arms

• Ability to frequently lift and or move up to fifteen (15) pounds

• Ability to occasionally lift and carry up to thirty-five (35) pounds (periodic lifting of supplies or

equipment)

• Ability to coordinate multiple tasks simultaneously

• Ability to travel to and from other health center sites and community locations

Work Environment: The work environment characteristics described here are representative of those an

employee encounters while performing the essential functions of this job. While performing the duties of

this job, the employee is regularly exposed to Health Center areas and general office environment. The

work environment is a clean, well-lit, well-ventilated, and temperature-controlled environment. The noise

level in the work environment is usually moderate. The hours of duty are lengthy and irregular.
