Job Summary:
We are seeking an experienced RN to fill the position of Clinical Manager for our home health agency in Richardson, Texas. The Clinical Manager will act as a liaison between the Director of Nursing and the clinical staff and will be responsible for providing oversight and direction to the clinical team. This position is critical to the agency's ability to provide quality care to our patients and meet regulatory requirements.
Key Responsibilities:
Act as a liaison between the Director of Nursing and the clinical staff
Ensure compliance with all state and federal regulations
Provide education and training to clinical staff on new policies, procedures and updates
Participate in recruitment and selection of nursing staff
Managing and overseeing clinical and administrative and recordkeeping functions
Responding to phone referral/inquiries in a timely, professional, and compassionate manner
Participate in on-call nursing schedule and providing direct skilled bedside nursing care
in patients homes as needed
Assure productivity requirements of a full-time, specific package, and contract staff.
Oversee all QAPI and utilization review activities, including timely initiation of
corrective action plans and controls.
Supervise clinical staff and their care coordination
Supervising contact for all disciplines involved with providing care to patients and
oversees the frequency of visits for the episode.
Consults as needed with the physician and physician office regarding patient care
Assuring the development, implementation, and updates of the individualized plan of care
Ensure thorough coordination and compilation of Quality Assurance the accurate
OASIS collection,
Ensuring RNs maintain the medication profile as current, ensuring lab values have been reported to the physician timely via office protocols, coordinating and leading weekly case conference and monthly case manager meetings are key responsibilities of this role.
Make supervisory visits and visits as necessary
Oversight of visits performed as needed and completing coordination of client care
Coordinating patient care
Coordinating referrals
Assuring that patient needs are continually assessed
Assist with New staff orientation and skills checkoff upon hire and yearly basis
Qualifications:
RN license
2-3 years of experience as a nurse in a home health setting
Strong working knowledge of OASIS documentation
ICD 10 coding knowledge and experience is a plus
Strong leadership and management skills
Knowledge of state and federal regulations for home health agencies
Strong communication and interpersonal skills
Able to work in a fast-paced environment
Kinnser experience is preferred.
Schedule:
8 hour shift
Monday to Friday
On call as needed
