Women’s Health Case Manager (LVN or RN)
The Women’s Health Case Manager is responsible for providing the full spectrum of case management services to pregnant patients during pregnancy and the post-partum period. Case management services include, but are not limited to:
initial patient intake
comprehensive visits
family needs assessments
service plans
referrals and related activities
follow-up
Essential job functions:
Submits prior authorizations to health plans for in-house services and referrals.
Delivers services in-person, by telephone, or by telehealth.
Works with the patient care team to assist with care and resource coordination.
Assists providers with care management by following up on procedure orders and labs.
Document all activities and submits codes for billing as appropriate.
Generating reports.
Required:
Registered Nurse (RN) license (Unrestricted) in the state of Texas required; Bachelor of Science in Nursing (BSN) preferred; or a Licensed Vocational Nurse (LVN).
At least 5 years related experience and/or training.
Basic Life Support (BLS) Certification.
Bilingual in English and Spanish.
